Referring Your Patient

COVID-19 Safety: To ensure the safety of patients and our team, only urgent and critical care patients are seen in person. All other patients are triaged accordingly for Telehealth appointments.

Accepting referrals for Obstetrical and Gynecologic care.

Referral Information

All patients seen in our office must be referred by a GP. Please ensure the referral request is concise and informative:

  1. Patient name, personal health number, date of birth, and contact details (phone number and email address). Please indicate the best way to get in contact with the patient, and if the patient has given consent for email communication with consulting physician.

  2. Date of referral, the reason for referral, and the urgency of the referral.

  3. Copies of relevant medical information, laboratory and imaging reports, information from previous consultations, and social information (language barriers, impairments, disabilities, gender identity, etc.)

  4. The expected outcome of this referral - E.g. Treatment, second medical opinion, transfer of patient care, etc.

Guidelines

Prior to sending the referral, please obtain patient consent, and ensure the patient is aware of the referral and its purpose. Please indicate the specific physician you would like to refer your patient to. If a specific physician is not indicated, the referral request will be rejected and returned to the referring doctor. Once the referral has been received, the patient's file will be reviewed by the consulting physician and triaged as soon as possible.

If the referral is complete and accepted: Our office will contact both the referring physician and the patient in order to provide information about the possible wait time, arrange a date and time for the upcoming appointment, and obtain any additional information.

If the referral is incomplete and rejected: Our office will contact the referring physician to indicate why the referral was rejected and what parts of the request were missing or incomplete.

Urgent Referrals

If the referral is considered urgent, please indicate this in the referral request. Please provide all necessary and important details, information, and lab work to support the urgency of this case. This will help to accelerate the referral process. Our office will do our best to provide an appointment for urgent patients as soon as possible.
